When this building project was designed and constructed it was known as the "Hospital Building for Apple Creek State School, formerly Apple Creek Institution for the Feeble Minded." The architect was Joseph A. Gattozzi of Cleveland, Ohio. The original blueprints the State of Ohio has were dated May 24, 1947.

The large brick building called McIntosh Hall was constructed between 1948 to 1949. When this State building project was designed and constructed it was known as the "Hospital Building for Apple Creek State School, formerly Apple Creek Institution for the Feeble Minded." The architect was Joseph A. Gattozzi of Cleveland, Ohio. The original blueprints the State of Ohio had drawn-up were dated May 24, 1947.

Local newspapers indicate it was closed down sometime in February of 2006 and the building was later demolished.
